To request a copy of your medical records for personal use from The Spine Hospital of Louisiana, you will need to fill out an Authorization to Release Medical Records form.  For your convenience, we offer this form online. WRITTEN AUTHORIZATION REQUIRED

HIPAA laws require that certain procedures be followed in order to release copies of a patients medical record, therefore, written authorization is required for us to release your medical records. These requests must be submitted directly (in person, fax, or by mail) to The Spine Hospital of Louisiana Medical Records department:
